CONCUSSION POLICY – MANDATED BY VYSA – Must be completed in order for HYS to be in compliance with our leagues regarding concussions

In the Spring we rolled out to all teams our concussion policy to be in compliance with our leagues.  This summer, VYSA our governing body rolled out additional guidelines regarding concussion awareness and tracking.  All parents & coaches are to review the policy and complete the online compliance documentation.  To find our policy, please click on the homepage of the website or on the link below.

In order to be in compliance with the new guidelines set, we ask that upon reviewing the documentation you and your player(s) complete the compliance documentation online.  Our goal is 100% compliance for all coaches, assistant coaches, managers, parents and players are all in compliance in accordance with the guidelines.

Discount Concussion Baseline Testing Offer

A discount has been arranged for your team members to receive a concussion baseline ImPACT test at the Fairfax Family Practice Comprehensive Concussion Center (FFPC3) D1-NOVA Testing Center in Chantilly.  This facility is located near the Target south of Route 50 near the Route 28 intersection.  More detailed directions will be sent to you upon registration.
DISCOUNT PROVIDED: $30 for baseline test which includes online concussion education for athlete and parent
To register for a baseline test and receive the discount, please create an account in the Concussion Education system by logging on to www.ffpc3.com and click on the “Concussion Education” button.  
If you are returning to our system and have previously set up an athlete dashboard, click on Athlete/Parent Login and enter the same email used previously and the Registration Key to access your child’s athlete dashboard.  If you forgot the Registration Key, please click on the “I forgot my Registration Key” and it will be sent to your email.  Once on the Athlete Dashboard, click on the button “Edit Organization Information”.  Here you can click on the option “I have a Team ID”.  Enter HYTS2015 and click on “Enter Team ID”.
Click on “Create a New Athlete User Account” if you are new to our program.  A Registration Key unique to each athlete will be created which is used to access your Athlete Dashboard. (An account must be created for each athlete if you have multiple children participating).
When prompted, choose “I Have a Team ID to Enter” and enter: HYTS2015 and click on “Enter Team ID”. 
A “Registration Key” will be displayed, please copy that key down for future reference.  There will be a unique key for each child you create an account for.  Proceed to log into the account using the email and Registration Key to access the Athlete Dashboard.  You can register for a Baseline ImPACT test, complete the Athlete Concussion Education and the Parent Concussion Education modules from there.
